• The information provided below is intended to assist you in preparing to enroll your child(ren).  Completing this information in advance of your school visit will expedite processing and allow for a smooth enrollment process.  Please note all forms are in PDF format and some allow electronic completion.  Click on each hyperlink to view, edit, and print each form.
    Checkbox Enrollment Form – Provides vital information regarding the child, parent/guardian, and siblings. 
    Checkbox Affidavit of Information Form – Under the provisions of Article XIII-A of Act 26 of 1995, prior to a student’s admission to any school entity, a sworn statement is required concerning the student’s prior disciplinary record.
    Checkbox Home language Survey – Must be completed by all families.  Used to determine a primary or home language other than English.
    Checkbox Emergency Information Card – The Emergency Information Card format is provided here to allow parents/guardians to complete the information in advance of the school visit.  All information will need to be recopied by the parent/guardian to an "Official Emergency Information Card" format upon registration at the school.
