Understanding the Application Process for Employment with Greensburg Salem School District
 
The following forms are necessary for your employment application to be considered for school board approval. Click on the links to be directed to the appropriate sites/forms. These forms are also available in the Human Resources Office, 1 Academy Hill Place, Greensburg. Greensburg Salem requires that all applicants' initial clearances are dated within 1 year of the date employment begins.

  • Three clearances (Click here to access the links for these required clearances)
    1. Act 114- FBI Federal Criminal History Record- Use code 1KG6XN when registering
    2. Act 151-  PA Child Abuse History Clearance
    3. Act 34- PA Criminal Record Check
  • Act 126 Recognizing and Reporting Child Abuse Mandated Reporting Training- Click on one of the FREE training courses and print your certificate of completion to submit with your application materials.
  • Act 168: PA Sexual Misconduct/Abuse Disclosure Release- Form must be submitted to current employers, former employers that were school entities, and former employers where the applicant was employed in a position that involved direct contact with children.
  • Act 24/Act 82: Arrest or Conviction Form 
  • School Personnel Health Record- A physical and TB test are required prior to beginning work, but not needed for application approval.
  • Additional requirements for teaching applicants:
    1. Pennsylvania Teaching Certificate
    2. PRAXIS Scores
    3. Official College Transcripts with Seal
    4. 3 External Letters of Recommendation (Cannot be from Greensburg Salem employees)
