Medication in School
The District recommends that prescription and nonprescription medications be administered at home whenever possible. However, the District recognizes that some students have health situations which require that they receive medication during the day. Therefore, the District will permit the administration of prescription and nonprescription medication under specific protocols and procedures as outlined below:
-
A medication administration form must be completed and signed by the physician and parent for any medication that may be administered in school throughout the school year. A new form and physician order is required at the start of each school year. Medication orders and administration forms must be dated on or after July 1st, 2024.
-
All over the counter medications require a physician's order and completion of a medication administration form.
-
No more than a 30-day supply of medication may be kept in the nurse's office.
-
All medication must be received in the original bottle. Medication in baggies or containers other than in the original labeled pharmaceutical container will not be administered. Over the counter medication should be in the original sealed package.
-
Prescription medication shall be delivered in its original packaging and labeled with:
- Name, address, telephone and federal DEA (Drug Enforcement Agency) number of the pharmacy.
- Student's name.
- Directions for use (dosage, frequency and time of administration, route, special instructions).
- Name and registration number of the licensed prescriber.
- Prescription serial number.
- Date originally filled.
- Name of medication and amount dispensed.
- Controlled substance statement, if applicable.
-
-
A responsible adult must bring the ordered medication to the nurse's office.
